Xbox 360 Most difficult achievement

Hey all! Just curious what you consider to be the most difficult achievement out there? It doesn't necessarily have to be one you've actually achieved (that certainly would make it sound all that much harder, right?). There's too many in my opinion, but off the top of my head, I would say "I'm a Golden God" on Super Meat Boy. It requires 100% of the game, and if you've played it then you know that...that's a challenge. I think I have 2 achievements total for Super Meat Boy?
 
Gears of War - Any of the Seriously Achievements (all major grindfests in the TENS OF THOUSANDS of matches and kills)

Dark Souls - Knights Honor (Acquire all Rare Weapons. Forget Prepare to Die, this is Prepare to Cry)

Ninja Gaiden 2 - Way of the Master Ninja (Beat the game on Path of the Master Ninja. Seriously, this game is just plain mean on Normal alone.)

CoD 4: Modern Warfare - Mile High Club (Beat the epilogue mission on Veteran. A lot of people know this one, you have 60 seconds to rush through a gauntlet of death.)

I definitely agree with all of these, they are insane, specially Ninja Gaiden 2's. I tried the MW one so many times but I just gave up and I don't plan to even bother trying for the other 2...

This one is actually ridiculously easy. At least it was for me. The game is what's difficult, not the achievements. The only issue with this achievement is the Channeler's Trident which has a ridiculously low spawn chance and I guess you also have to beat the game 3 times (which came naturally to me and you also have to beat it 3 times for the Miracle achievement). I found Dark Souls' achievements to be very simple... I'm now hunting them again on the PC version, by the way, heheh.
 
Those do sound tough. Yeah, I forgot about the Gears' games grindfests. I anticipate achieving those in, uh, maybe 10 more years provided the servers for online matches stay going that long. Hahaha.

Also, on that note too--any game that no longer offers on-line servers, or has no on-line playerbase, yet has achievements that require both of those things... is impossible now. Sadly, I feel like that's growing more common.
 
I'm going to go with Left4Dead's finishing all campaigns on Expert difficulty. My buddy and I have never done that. We've tried over and over and eventually it just gets old. You need to have four players that are on top of their game and make it through the finales. Left4Dead is older now and with Left4Dead 2 out I doubt I'll retry this one.

Another one from that game is no friendly fire for the entire campaign. The thing about Left4Dead is that even when you play on easy and even when you are careful and walk slowly through the levels trying to not make a mistake. I've made it so far as level 5 of Dead Air and then...whoops shot Bill.

I don't know if it's necessarily difficult, but just the fact that Half Life 2 has an achievement that is worth only one point, has put me off from ever touching it on The Orange Box. There is no way that I would be able to complete The Orange Box and that one point would drive me insane. So, no go on my gamertag.
 
The only thing coming to my head right now is trying to beat the whole campaign on veteran difficulty on Call of Duty: Black Ops. I'm pretty sure some of you breezed through the whole campaign on veteran difficulty, but this was only of the most hardest achievements I can imagine. It's not necessarily difficult, but it just takes way too much patience from one person. It took me so many times and starting from last checkpoints I was about to go crazy. I raged so many times I probably lost 5 years of my life.
