Xbox 360 Pirated Copes of Halo 4

I just came across an article released about 2 months ago - before the release of Halo 4 - that said Microsoft had been permanently banning users for playing pirated copies of Halo 4 before the game had even been released. How would these people have gotten their hands on a pirated copy of the game? Was someone on the inside making illegal copies of the game and selling them?

That would be my guess. I don't know else how a member of the general public could have gotten their hands on a game that hadn't gone on sale yet, ya know?
